Fixes

• [Interactivity] Correct answers are now graded as passed on quizzes with a pass threshold, including quizzes without pass actions.
• [Interactivity] Quiz tracking events now report accurate values: passed, num_correct (correct questions) and num_answered (all answered questions).
• [Interactivity] Grading matches the legacy player: a wrong extra option in a multiple-answer question lowers the score, and questions without a correct answer accept any answer.
• [Interactivity] The custom passing mark only applies when "custom threshold" is enabled; otherwise 51% is used.
• [Interactivity] Gated quizzes: the Cancel button is hidden while the gate is active, so viewers can no longer get stuck.
• [Interactivity] Gated quizzes with "Reset questions": a wrong answer now jumps back as configured and the quiz stays gated until it is passed.
• [Interactivity] A quiz that fails and resets its questions is shown again even when the project shows questions only once.
